As we all know, the complications of chronic kidney disease and the associated risk factors can be quite significant. They often lead to excessive morbidity, frequent hospitalizations and premature mortality. At Inland Empire Nephrology, our nephrologists, advanced practitioners, nurses, dietitians and support staff recognize that a major part of protecting public health rests with you and your staff. You care for patients before they come to us, and our role is to support you. As the primary caregivers who make the initial diagnosis of at-risk patients, you play a vital role. And you make a difference.
When To Refer a Patient
These are some of the conditions that require nephrology intervention:
- Unexplained proteinuria quantitated greater than 500 mg/day.
- Nephrotic syndrome (Heavy proteinuria, edema, hypoalbuminemia, hyperlipidemia, +/- hypertension)
- All patients with GFR < 60 and GFR declining.
- Calculated GFR < 60 and concomitant proteinuria, hematuria and/or pyuria.
- A patient expected to need dialysis
- Incidental diagnosis of polycystic kidney disease on CT, US or MRI
- Patients with the combination of proteinuria and hematuria or pyuria (even when proteinuria is low-grade). See note below.
- Patients who have a kidney transplant
This list is not absolute, and there are other conditions that you may desire to consult with us about, including:
- Sudden worsening of blood pressure in a previously stable patient
- Severe and/or uncontrolled hypertension:
- Unexplained hematuria
- Kidney stones
- Medullary sponge kidney
- Electrolyte abnormalities such as hyponatremia, hypercalcemia, acid base disturbances, and hyperkalemia, among others.
